HEALTHCARE FOR RE-ENTRY VETERANS (HCRV)

A VA program designed to help re-entering veterans with healthcare concerns. The program specializes in outreach and pre-release assessments services, referrals and linkages to medical, psychiatric, and social services, including employment services, and short-term case management assistance.

This VA site contains guides and information on re-entry resources for all fifty sates and the District of Columbia. A great place to start and to learn about the resources available after your release.

The National Coalition for Homeless Veterans produced this guide to help transitioning veterans. It contains much useful information and resources for veterans and non-veterans.

INCARCERATED VETERANS OUTREACH INITIATIVE

The Incarcerated Veteran Outreach Initiative is a partnership between the US Department of Veterans Affairs Veterans Benefits Administration, the New York State (NYS) Department of Correctional Services, the NYS Department of Parole, and the NYS Division of Veterans Affairs. This initiative was developed to meet the needs of formerly incarcerated veterans with histories of serious mental illness and/or substance abuse who are homeless.

VETERANS’ INCARCERATED PROJECT (VIP)

VIP is a “fast track” effort to help veterans recently released from, or currently incarcerated within, the King County (Washington state) Correctional Facility system. A full array of on-site and referral sources are available, including services for family members.
